EACH SUPPLIER BEFORE DELIVERING WASTE TO WASTE TREATMENT PLANT SP. Z O.O. IS REQUIRED TO GENERATE A BDO CARD, I.E. A WASTE TRANSFER CARD IN THE ELECTRONIC DATABASE ON PRODUCTS AND PACKAGING AND WASTE MANAGEMENT.

Procedure for receipt of medicines

SCHEME OF CONDUCT

Secured medicinal products, together with a medicinal product transfer protocol signed by the Pharmaceutical Inspector, which also includes the signature of the responsible person, can be delivered to the Waste Disposal Facility by the entity or entrepreneur every third Wednesday of the month from 9:00 to 11:00 am.
The package, secured and signed, is received by an authorized person: checking the security and seals, confirming receipt with a signature on the transfer protocol. One copy of the protocol is archived at the Waste Disposal Facility, the second is retained by the person representing the entity or entrepreneur, while the third, the entity or entrepreneur transfers to the Silesian Provincial Pharmaceutical Inspectorate in Katowice, 15 Raciborska Street.
Then the authorized person weighs the secured package and immediately transfers it to disposal , as part of the next loading of the disposal line.
From the disposal performed, the person representing the entity or entrepreneur draws up a protocol in three identical copies in accordance with §4 paragraph 7 of the Regulation of the Minister of Health of February 27, 2012 on detailed conditions and procedures for handling narcotic drugs, psychotropic substances and precursors of category 1, their mixtures and medicinal products, spoiled, adulterated or expired, containing in their composition narcotic drugs, psychotropic substances or precursors of category 1
The protocol of disposal shall be signed by: a) a person authorized by the Director of the Disposal Facility b) a responsible person or a representative representing the entity or entrepreneur surrendering medicinal products for disposal.
The following prices apply for the disposal services of medicinal products, drugs, narcotics and other materials requiring commission destruction:
acceptance and disposal of drugs up to 3 kg - a flat fee of PLN 125.00 + VAT, and for each kilogram above the weight covered by the flat fee - a fee of PLN 37.04 + VAT;
acceptance and disposal of drugs, narcotics and other materials for commission destruction up to 3kg - a flat fee of PLN 148.15 + VAT, and for each kilogram above the weight covered by the lump sum - a fee of PLN 37.04 + VAT.
The above-mentioned price rates apply to each separately issued waste collection protocol.
